Eligibility Requirements

To qualify for the IMLC, physicians must meet the following criteria:

  • Hold a full, unrestricted medical license in a Compact member state (State of Principal License, SPL).
  • Meet at least one of the following criteria for the SPL:
    • Primary residence in the SPL.
    • At least 25% of medical practice in the SPL.
    • Employer located in the SPL.
    • SPL used as the state of residence for U.S. federal income tax purposes.
  • Graduation from an accredited medical school.
  • Completion of accredited graduate medical education (e.g., ACGME or AOA).
  • Passing required exams (e.g., USMLE, COMLEX-USA) within specified attempts.
  • Holding a current specialty certification.
  • No history of disciplinary actions, criminal history, or controlled substance actions towards their medical license.
  • Not currently under investigation.

Application Process

  1. Prequalification: Verify that you meet all eligibility requirements.
  2. Apply Online: Start the application on the IMLC website, select your SPL, complete the form, and pay the $700 non-refundable fee.
  3. Background Check: Submit fingerprints for a national criminal background check as required by your SPL.
  4. Receive Letter of Qualification (LOQ): If you qualify, you will receive an LOQ from the SPL.
  5. Select Additional States: Choose the additional Compact member states where you wish to be licensed and pay the state licensing fees.
  6. Issuance of Licenses: The medical boards of the selected states will issue your licenses, usually within a few days.
  7. Compliance with State Requirements: Comply with any additional requirements of the issuing states, such as jurisprudence exams or legal work authorization documentation.
  8. Maintenance and Renewal: Maintain your SPL status and renew each state license as required.
